Interprofessional Care – The Way Forward

What is interprofessional care (IPC) and how can you make it work for your organization? Attend this session to uncover a number of innovative ideas on how to develop and implement IPC strategies in the long term care and community sectors.
You will examine the similarities and differences between conventional care teams and care teams that function within an interprofessional client-centred model of care. Find out the connection between interprofessional education and interprofessional practice. Learn about the barriers, opportunities and critical resources you will need when introducing IPC into an organization. Draw on the varied experiences of others using IPC and identify how emerging new care models, including MDS and ministry regulatory requirements, can be used to support a model of IPC.
Objectives:
Understand the operating definitions of interprofessional care.
Learn about the key success indicators for introducing interprofessional care.
Identify opportunities for internal and external partnerships that support interprofessional care and take away a preliminary game plan for implementing it in your organization.
Amy Go is the executive director of Yee Hong Centre for Geriatric Care in Markham where she oversees a 200-bed long term home serving Chinese and South Asian Canadian residents.
Rivie Seaberg, RN, BScN, MPM, is the director of George Brown College’s Centre for Health Sciences. She also oversees the integration of interprofessional health sciences education principles into the centre’s programs of study.
Dr. Joshua Tepper is a family physician and an assistant deputy minister with the Ministry of Health and Long-Term Care, Health Human Resources Strategy Division. (invited)
