Sharing successes and innovative approaches to services for seniors is what the OANHSS Awards program is all about. In 2008 we had some truly unique and exciting submissions reflecting the creative things going on every day in member organizations. Below is a summary of the entries received for the following awards:

Luther Village on the Park

Lutherwood Diabetes Management Support Program

 

Lutherwood’s Diabetes Management Support Program is an interactive web based service that connects patients and providers online for effective health management.

 

In partnership with MedManager and MicoHealth, the Lutherwood Diabetes Management Support Program educates and empowers residents to improve their diabetes health management, quality of life and allows improved access to health care services.

 

Through access to a user-friendly computer program, residents with diabetes can monitor their health on a regular basis and their health care team can easily and conveniently coach and educate the resident through the internet in order to ensure optimum self management. The program is accessible 24 hours a day from any computer with an Internet connection.

 

The program empowers the resident to be more responsible for their own health management and offers a number of features including goal setting and tracking tools; automatic blood glucose meter readings storage and viewing; diet, medications and exercise records; and more.

 

This diabetes management program is easily operated, can be customized for other seniors’ settings and can be applied to other chronic diseases that affective seniors, such as Chronic Obstructive Pulmonary Disease (COPD).

 

Lutherwood is currently implementing a Cardiac Wellness Program which will be piloted and work in very much the same way.

Parkwood Mennonite Home

Staff Education Learning Passport Program

 

In 2007, the management team at Parkwood Mennonite Home decided to change the way they approached mandatory training for staff. Their previous system had been to hold monthly one-hour in-services, which were difficult to schedule due to the variety of shifts worked in the home. Attendance ranged from fair to poor.

 

Given that training sessions are mandated by the Ministry of Health and Long-Term Care and are also vital to keeping staff informed on new developments, the home determined that it needed to look at a new delivery model.

 

Parkwood developed an annual half-day paid education program for all staff, which focused on required training as well as home identified topics. It consisted of 12 stations, or sessions, covering topics such as abuse and harassment, RAI/MDS, palliative care, restraints, infection control, WHMIS, fire safety and emergency codes and others.

 

The sessions were led by Parkwood managers and staff as well as by various community partners. Every staff member received a learning passport listing the 12 stations that they had to visit. The presenters had 20 minutes to go through their material and at the end of each session they signed each passport and added a sticker to verify attendance. By the end of the training sessions, all spots in the employee’s passport would be filled and were collected to be placed into their personnel file.

 

Two full days were set aside for the training and staff were scheduled to attend either the morning or afternoon session on one of the days. Having 12 stations meant that the groups were small, in most cases a maximum of five individuals, which allowed for more interaction and increased the comfort level of participants.

 

Feedback on this new education format has been overwhelmingly positive and the home was very encouraged by the results of a follow up evaluation showing how much information was retained.
